Support Worker - Aged Care (Area - Laurieton to Kempsey)

The Home Care Package (HCP) team provide individually planned and coordinated suite of support and services designed to assist older people who need support to remain living independently, in their own homes. 

• Casual position covering the area spanning from Laurieton to Kempsey.
• Office based in Port Macquarie (Birpai Nation, Birpai People). MUST be living in this LGA.
• Open to experienced and new Support Workers with a min Cert III in Aged Care
• 25% casual loading + Not-for-Profit Salary Packaging to maximise your take home pay

About the role 

We are looking for a Support Worker to work casually across our Home Care Packages program. Home Care Packages responsibilities:

• Deliver high-quality individualised support and assist our older adult clients, supporting them to maintain and develop their independence so that they can lead enriching and fulfilling lives.
• Follow and implement individual service plans, healthcare, and positive behaviour support plans.
• Provide companionship, conversation, and emotional support and support with personal care in the client's home.
• Assisting with transportation to attend appointments.
